Filling out the pdf Forms- Please Read
Download the applicable Intake Form pdf. Open the pdf with Adobe Reader (Get Adobe Reader here). You should be able to fill in your answers, save the completed form, and then send the pdf back via email as an attachment. [Please do not use EchoSign to send the Form back, as the document will not come through that way.]
Or if you prefer, you can fill out the Form, (print it) and scan it, & then send the scanned image via email.
